Molecular hydrogen and oxidative stress: an honest look

Search for information on molecular hydrogen and oxidative stress and you will find two camps. One promises that hydrogen will transform your health; the other dismisses the whole field as nonsense. The honest answer — the one we try to give at our small clinic in Somerset — sits somewhere in between: a genuinely interesting hypothesis, a growing pile of early human studies with mixed results, and some large unanswered questions.

This article is our attempt to lay all of that out in plain English, including the parts that are inconvenient for a business like ours. We would rather you read this and decide not to visit than book a session on the strength of claims the science cannot support.

What oxidative stress actually is

Your cells make energy constantly, and the process is not perfectly tidy. It produces reactive oxygen species — often called free radicals — unstable molecules that can react with proteins, fats and DNA in ways they shouldn't.

Free radicals are not simply villains, though. Some act as messengers in normal cell signalling, and your immune system uses them deliberately. Your body also runs its own antioxidant defences to keep everything in balance.

Oxidative stress is the name researchers give to the state where that balance tips — more reactive molecules than your defences can comfortably handle. It is studied in relation to ageing, intense exercise and general cellular wear and tear. It is a normal part of being alive, not a diagnosis, and nobody should be frightened into "fixing" it.

Hydrogen as a selective antioxidant: the 2007 hypothesis

The modern research interest in hydrogen largely traces back to one paper. In 2007, Ikuroh Ohsawa and colleagues published a study in Nature Medicine proposing that hydrogen gas acts as a selective antioxidant.

In experiments on cultured cells, they reported that hydrogen reacted with the hydroxyl radical — generally considered the most destructive of the reactive oxygen species — while appearing to leave the milder, signalling-type radicals alone. In rats, breathing hydrogen gas reduced markers of brain injury after blood flow was restricted and then restored.

That selectivity is what made the paper interesting. Ordinary antioxidant supplements are indiscriminate; a molecule that only mopped up the most damaging radicals, if the finding held, would be unusual. Hydrogen is also the smallest molecule there is, which lets it diffuse easily through tissue.

The limitations matter just as much. This was a study of cells and rats, not people, and "selective antioxidant" remains a hypothesis rather than settled fact — later reviews have argued that radical scavenging alone cannot explain everything reported in hydrogen studies, and that other biological pathways may be involved. What molecular hydrogen research shows in humans

Since 2007 the field has grown quickly, but not evenly. A 2015 review counted 321 original research articles on molecular hydrogen — and noted that roughly three quarters of them were studies in mice and rats. Human evidence is the smaller slice, and it deserves to be looked at squarely.

Exercise and recovery studies

A 2012 pilot study followed ten young footballers in a double-blind crossover design. Drinking hydrogen-rich water before exercise was associated with lower blood lactate during hard effort and less early decline in muscle performance — but the blood markers of oxidative stress did not significantly change, and the authors were careful to call their results preliminary.

A 2024 systematic review and meta-analysis pooled six studies with 76 participants in total. Across them, hydrogen supplementation did not significantly reduce a common marker of exercise-induced oxidative damage compared with placebo, though it was associated with a modest improvement in one measure of antioxidant capacity. The authors concluded that more rigorous studies are needed.

Everyday markers in healthy adults

A 2020 randomised, double-blind trial gave 38 healthy adults either hydrogen-rich water or plain water — 1.5 litres a day for four weeks. The headline oxidative stress markers ended up no different between the groups. In participants over 30, one measure of antioxidant capacity rose more in the hydrogen group, and blood cell analyses suggested reduced inflammatory signalling — interesting hints, but from a single small study, and subgroup findings are precisely the kind of result that needs replicating before anyone builds claims on it.

That is the honest pattern across the human research: small samples, varied protocols, some measures moving and others staying flat. This is what early-stage science looks like from the inside — promising enough to keep researchers interested, nowhere near enough to justify the bolder claims you may see elsewhere.

What the research doesn't tell us yet

Being straight about the gaps matters more to us than the highlights. Here is what remains genuinely unknown:

  • Dose and delivery. Studies use hydrogen-rich water, inhaled gas and hydrogen baths, in widely different amounts and timings. Nobody yet knows what an optimal protocol looks like, or whether one exists.
  • Duration. Most human trials run for days or weeks. Long-term effects, good or bad, simply have not been studied properly.
  • Who, if anyone, benefits most. Athletes? Older adults? The over-30 finding above is a question, not an answer.
  • Whether the changes matter. A blood marker shifting on a lab report is not the same as a benefit you can feel in daily life. That link has not been established.

For that reason we will say this plainly: hydrogen therapy is not a medical treatment. It does not diagnose, treat, cure or prevent any illness or medical condition, and nobody should tell you otherwise. So why do we run a hydrogen clinic at all?

It is a fair question, and we ask it of ourselves regularly. Our answer comes in three parts.

First, a session is gentle and non-invasive. You sit comfortably, you breathe, and in truth you will not feel anything happening — we have described exactly what happens in a hydrogen therapy session, disappointment included, because we think you should know before you come.

Second, the direction of the research interests us. The selective antioxidant hypothesis is biologically plausible, the number of human trials keeps growing, and we would rather offer sessions transparently — caveats and all — while the science matures, than pretend the caveats do not exist.

Third, and most importantly, our philosophy was never about illness. We opened the clinic to support natural recovery and everyday vitality — the same reasons we set out in why we opened a hydrogen clinic in Somerset.

We would rather talk than sell. If you ask us whether hydrogen will fix a health problem, we will tell you the truth: that is not what we offer, and the research does not support anyone who promises it.
